[No one, least of all the pet, likes the Cone of Shame. But it’s necessary…and up to you to make sure your pet keeps it on]
A common conversational topic among any group of veterinary personnel in any setting – work, conferences, even dinner – is clients. It makes sense. Although our work is nominally all about the animals it’s nearly impossible to have an animal without a client attached. A huge part of my job as a technician is interacting with clients in a variety of circumstances, good and bad. I take an initial history when you come in for an exam, discuss estimates for care, review medications and some test results, discuss homecare, and generally act as a resource and go-between for you and your veterinarian. I understand that our experiences together are not always easy; we see you during the panicked and stressful times of emergency, during financial hardship, family troubles, and at the end of your beloved pet’s life no matter when that time comes. I will never judge your sadness, your anxiety, your frustration. My job is to advocate for your pet. But with that said, there is one thing that comes up again and again in discussions, one thing that I hope all clients can keep in the back of their minds as they interact with their local vet services…
Dear Clients,
Please be honest with your veterinary team.
This may seem like a no-brainer, a silly peeve because after all, I’m always honest! Why wouldn’t I be? But please know, in my time in the veterinary field I have run across clients not being entirely honest with me every single day, and often multiple times. I don’t think people are always aware of it, either – there may be a feeling of well, this isn’t relevant or why would they need to know that or more worryingly, an element of distrust: if I tell them, they’ll judge me. They won’t treat me or my pet with integrity. I can’t speak for every veterinary team member in the world but I can assure you that for the vast majority of us this is not only not true, but can have serious adverse effects on your pet’s care. I’ve compiled a list of the four things I’ve found clients most struggle to be honest about, and tried to explain why we desperately need the truth in these scenarios.
1) Tell us what happened to your pet. I get it, I do. It can be embarrassing to realize that something you’ve done or given to your pet may have made them sick. You may worry that we’ll judge you, berate you, or in certain cases, report you to authorities. What you need to understand is that for us, your pet comes first. Always. When we ask the hard questions – Is it possible that Fluffy ingested any drugs, prescription or otherwise? Have you given Buster any new treats or foods lately? How long has it been since Whiskers last ate? – it’s because we need to know these answers in order to properly treat your pet. If you don’t tell us that Fluffy might have gotten into your marijuana, we won’t know to treat for suspected marijuana toxicity and will instead need to perform several diagnostic tests and potentially prescribe inappropriate treatment if our findings aren’t conclusive. We’re not going to turn you in – we’re going to make sure that Fluffy is OK. Many drugs, prescription or otherwise, can have serious and potentially fatal effects for pets but the faster we’re able to start treatment, the better the outcome is likely to be. If Buster was giving you the puppy eyes and you gave him two Big Macs the day before he started vomiting and having diarrhea, that’s important information. We won’t judge you (the only treats my fantastically picky cat will eat are chicken hot dogs and salmon skin) and we’ll know right off the bat that it’s more likely a gastroenteritis or pancreatitis than a toxicity or foreign body. If Whiskers hasn’t eaten in a week but you haven’t been able to bring her into the vet for whatever reason and you tell us that it’s only been a day or so because you feel embarrassed or ashamed, we’ll treat it as significantly less of an emergency and potentially miss lifesaving treatments. Your pet’s health is in your hands and your voice; a thorough, accurate history is one of the most useful diagnostic tests we have.
2) Tell us your financial concerns up front. Reputable veterinary services should be making sure that you are always aware of what things will cost. If you are cost-concerned or have a certain limit, please make that one of the first things you tell us. Once again, we won’t judge (I can guarantee you that everyone at the office you speak to has been in similar situations with their own pet and struggled with finances) and it means that we can try and tailor your animal’s health plan to your financial abilities. We can give you options for financing companies or, more rarely, clinic payment plans (this is not usually done because of the sheer amount of clients who do not follow through) and we can ensure that whatever tests we perform and medications we give are within your price range and that if they are not, we can counsel you on other options. If you are given an estimate, please tell us if the high end is not possible for you; it’s there for a reason. Being honest about financial concerns will make things easier for you and your pet in the long run, and it will keep your veterinary clinic happy with you!
3) Tell us if you can’t comply with homecare or give medications. I can name at least ten cases off the top of my head where a surgery has failed or an animal has worsened due to the owner’s inability to give medication appropriately or keep up with the necessary at-home care. Our goal is always a healthy, happy pet – if you know that your job means you can’t give eye medications four times daily, or your housing situation means that you can’t keep your dog confined, or your cat’s attitude means that there’s no way he can get pills twice a day – we need to know so that we can try to find a way to make treatments work for you! There are sometimes medication options such as long-acting pain control or antibiotics for difficult-to-pill animals, mild tranquilizers for animals which need to rest but can’t be confined, and various other strategies to ensure your pet gets the necessary care to heal well. As always, there is no judgement here. We just want to make sure that we won’t see Fluffy again for something we could have prevented. This is also why we send pets home with Elizabethan collars or medical shirts to cover wounds/incisions; not because we like to torture your poor dog as he bounces off every wall in the house, but because if he licks that wound the best case scenario is that it will heal much more slowly, and the worst care scenario is tissue damage that may need surgery. No one likes the Cone of Shame, but it’s our responsibility to keep our pets from hurting themselves!
4) Tell us if your pet may be aggressive. This is a huge one and probably one of the most important on the list; if your pet has a history of aggression, severe anxiety, fear-biting, stranger aggression or anxiety, or you even just suspect they won’t do well with us, you need to make this known to everyone who may handle your animal. Not only will we not judge you, but we’ll probably thank you on bended knee for helping us not get hurt. Animal bites and attacks are worryingly common in veterinary clinics. It’s a loud, stressful, smelly place for your pet, full of emotions and sounds and strange people poking them in uncomfortable places — certainly enough to send even the most well-balanced pet into fight-or-flight mode! If we know your animal may lash out or suspect that it’s a possibility, we will take appropriate steps to protect ourselves. Muzzles and chemical restraint (sedation) are our two best friends when it comes to difficult animals and neither should be shied away from; muzzles allow us to physically examine your pet and perform treatments with less restraint, while sedation allows the same with the added benefit of allowing your pet to feel calm and relaxed. Our jobs are dangerous as it is; please help to keep us safe and understand that when we suggest a muzzle or sedation for your animal, it is not a judgement on you or your pet’s manners – it is ensuring that everyone (including your pet) is kept safe and happy.
I know I’ve said it several times, but my first priority in a clinic is the health, happiness, and safety of your animal. I will advocate for them at all times and treat them the same way as I’d treat my own (maybe without the chicken wieners), but I can’t give your pet the best care possible without honesty from you. Veterinarians are here to help – please help us to do our job!
